CEDI Reminder – Completing CEDI Enrollment Forms

All CEDI enrollment forms are completed and submitted online through the CEDI Web site at https://www.ngscedi.com.

Guided Enrollment Instructions are available upon initiating the enrollment process as well as under the Help option accessible throughout each step of the process.

Below describes the top reasons for rejected CEDI enrollment forms and guidance on preventing and/or resolving these rejections.

  • Signee is not authorized to sign on behalf of the supplier

The authorized or delegated official’s name entered in the DME Authorized Supplier Signature field will be verified against PECOS. The DME Authorized Supplier Signature must match the authorized or delegated official’s name as it appears in PECOS to be accepted. Access PECOS at https://pecos.cms.hhs.gov/pecos/login.do External Link. ​​​​​​​to verify how the authorized or delegated official’s name is listed.

Important: A full name is required. Please do not use initials or nicknames when signing the forms, as the Enrollment request will be rejected.

CEDI Enrollment will only accept and process requests signed by the authorized and/or delegated official of the facility.  Clearinghouses, Billing Services and/or other Third Party Entities are NOT ALLOWED to sign enrollment forms and must not submit them on behalf of the supplier.

  • Supplier Name Does Not Match Medicare Database

Supplier/Provider Name received on EDI Enrollment Agreement does not match the name on the Medicare Database. The Supplier Name must match the Legal Business Name as it is in PECOS to be accepted.

Please allow up to 5 business days for CEDI to complete your enrollment request(s).
