CEDI Enrollment Forms are Currently Unavailable

The online CEDI Enrollment Forms are currently unavailable on the CEDI Web site (www.ngscedi.com). CEDI is working diligently to resolve the problems with the online enrollment forms and will have them corrected by September 17, 2008. A Listserv notification will be sent as soon as they are available.

DME MAC suppliers that need a CEDI Enrollment form, prior to September 17, 2008 should contact the CEDI Help Desk at 1-866-311-9184.
